Medication Therapy Management

Since 2006 PerformRx Connects has been providing Medication Therapy Management (MTM) services on behalf of multiple Health Plans.

PerformRx Connects offers a personalized pharmacist consultation with every enrolled patient, while engaging physicians and providers to optimize drug therapy and enhance Medication Therapy state management programs. This interactive service is provided to members to improve their overall healthcare and decrease cost.

MTM is provided by PerformRx pharmacists in coordination with other qualified healthcare professionals, such as case managers.

Patient and provider recommendations and interventions address medication issues such as:

  • Medication adherence
  • Therapeutic duplication
  • Drug-drug and drug-disease interactions
  • Appropriate dosing
  • Polypharmacy
  • Generic substitution opportunities
  • Therapeutic substitution opportunities
  • Medication monitoring opportunities, i.e. lab values related to medication use
  • Patient satisfaction

PerformRx has aligned its MTM Program with APhA and the NACDS Joint Initiative. March 2008.

Core Elements of an MTM Service Model Version 2.0.
  • Medication Therapy Review (MTR)
  • Comprehensive Medication Review/Record (CMR)
  • Medication Action Plan (MAP)
  • Intervention and/or Referral
  • Documentation and Follow-up

PerformRx MTM pharmacists have received MTM National Certification training from the American Pharmacists Association and the American Society of Consultant Pharmacists.

The clinical consultation process includes:

  • Medication Therapy Review (MTR) to identify medication related problems
  • Outreach call to patient
  • Medication Action Plan (MAP) provided to each member after consultation
  • Interventions and/or referrals called, mailed or faxed to health care professionals
  • Patients are followed to educate, re-evaluate and document outcomes
